NY STATE CHILD PERFORMER PERMIT

The NY STATE DEPARTMENT OF LABOR requires that all children working in professional film, television, modeling and theater have a Child Performer Work Permit.

They have made this process easier by providing videos for parents.

PLEASE WATCH ONE TIME THROUGH BEFORE DOING ANYTHING

NOTE : You can only get this the FIRST TIME YOU APPLY ... it is a one time first time permit. Be sure your shoot fits within those 15 days or be sure to begin the 12 month process right away so you don't have a lapse in your permit.
You will need:
Your child's social security number

You will need :
A TRUST ACCOUNT (and paperwork that proves you have one in your child's name)
BIRTH CERTIFICATE
SS#
PARENT ID
DOCTOR'S NOTE
SCHOOL APPROVAL 
PERMIT APPLICATION
AND at least TWO WEEKS TO PROCESS

It would be amazing if you could start working on your child's permit now so they have the 12 month permit when we call them to work.
It must be renewed every 12 months so be sure your child's is up to date.  It takes 2 weeks... to process them...
